The event was moderated by Dr. Anita Angelova, a specialist in obstetrics and gynecology. The discussion was attended by Dr. Mariela Vasileva, chairwoman of the society and specialist in general surgery, and Assoc. The project \"Patients Pathway for Breast Cancer\"<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-78a70bd elementor-widget elementor-widget-text-editor\" data-id=\"78a70bd\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<span class=\"citation-318 citation-end-318\">The project started more than an year ago with support from America For Bulgaria Foundation.<\/span>.\n\n<span class=\"citation-317 citation-end-317\">It includes different initiatives by the association<\/span><span class=\"citation-316 citation-end-316\">such as\u00a0<a href=\"https:\/\/bbca.bg\/en\/patients-pathway\/\" target=\"_blank\" rel=\"noopener\">an interactive map on the association's website<\/a>that contains sections on prophylactics, screening and diagnostics<\/span>. <span class=\"citation-315 citation-end-315\">Dr. Vasileva emphasized that the goal of the map is to direct patients to various talks and podcasts to obtain as much information as possible about the patient's journey<\/span>. <span class=\"citation-314 citation-end-314\">The association has translated the European Society of Medical Oncology's recommendations for patients, which are available as a free booklet on the website<\/span>.\n\n\u0412 \u0441\u044a\u0442\u0440\u0443\u0434\u043d\u0438\u0447\u0435\u0441\u0442\u0432\u043e \u0441 <a href=\"https:\/\/lymphoedema-bg.org\/\">Bulgarian Association Lymphedema<\/a> we have published two books on lymphedema after treatment for breast cancer and gynecological cancer, co-authored by Dr. Miroslava Chortova and Assoc. Prof. Yordanov.\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-21cb673 elementor-widget elementor-widget-heading\" data-id=\"21cb673\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">2. Diagnostics and staging<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-7de2084 elementor-widget elementor-widget-text-editor\" data-id=\"7de2084\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Bulgaria lacks a national breast cancer screening program and generally recognized national guidelines for diagnosis and treatment.<\/p><p>Dr. Vasileva explained that the patient's journey begins with a preventive examination or when a breast mass is detected by the patient herself. In the case of a suspicious mass, a triple evaluation is performed: clinical examination, imaging studies and a core needle biopsy (the so-called \"core needle biopsy\").<\/p><p>Biopsy is extremely important because removing the tumor before diagnosis may deprive the patient of effective treatment.<\/p><p>Staging is determined by imaging studies and histological analysis of the biopsy material, which includes the type of tumor, its differentiation, and the status of the receptors for estrogen, progesterone, HER2, and the proliferative index Ki-67.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-3244ec7 elementor-widget elementor-widget-heading\" data-id=\"3244ec7\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">3. Prof. Yordanov emphasized that patients in Bulgaria are not sufficiently informed, as the information is often not based on global guidelines. He categorically stated that patients should actively participate in decisions about their treatment.<\/p><p>Ovarian suppression, which is required in young women with hormone-sensitive breast cancer, was discussed.<\/p><p>Suppression can be medical or surgical (removal of the ovaries).<\/p><p>The surgical method is suitable for patients with metastatic cancer or with a BRCA1 mutation.<\/p><p>The advantages of medical suppression are that it is reversible and allows for the restoration of menstruation and preservation of reproductive function.<\/p><p>From a financial point of view, Assoc. Prof. Yordanov noted that surgical castration becomes more cost-effective after two years compared to drug suppression.<\/p><p>As the first choice for non-metastatic patients, current guidelines point to drug suppression, while in metastatic patients the better option is surgical removal of the ovaries.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element
